What is a Care Review?

An integral service in the GPEP program is called the Care Review Process (CRP) which is a comprehensive, interdisciplinary intervention which provides an individualized care planning process by focusing on the needs of residents who present with challenging behaviours. The goal of the CRP is to examine the cause or influence behind behaviours and develop a plan of care with staff. The outcome of this process to support person centred care in an active learning and collaborative manner with an interdisciplinary team. The CRP consists of three internal processes:

• introduce the GPEP Model to provide a context for understanding behaviours and to subsequently develop care strategies
• provide timely education for staff based on needs identified during the CRP
• evaluate the effectiveness of the CRP which includes addressing issues around sustainability and transfer of knowledge into practice.
